AI that calculates an individual’s general health risks based on scientifically validated machine learning algorithms utilizing lifestyle, demographic, environmental, and SDOH data. AXL provides a risk index from positive to negative at both the individual person and population level including the stack ranked medically peer reviewed risk factors that drove the individual’s health risk score.  In addition, using predictive analytics AXL’s AI can identify both rising risk and low risk individuals and populations for diagnosed as well as undiagnosed patients and members.

Chronic Care Management Risk

AXL’s Chronic Care Management Risk Indexes calculate an individual’s chronic disease risk based on scientifically validated machine learning algorithms utilizing lifestyle, demographic, environmental, and SDOH data. AXL provides a risk index from positive to negative at both the individual person and population level including the stack ranked medically peer reviewed risk factors that drove the individual’s chronic condition risk score.  Currently AXL’s AI supports the following chronic conditions: Cardiovascular Disease (CVD), Diabetes, and Kidney Disease – End Stage Renal.  Soon AXL will support Respiratory as a chronic condition index as well.

SDOH Risk

This algorithm can pinpoint a person’s highest priority need utilizing nearly 600 SDOH based individual data elements to calculate a health disparities index at the individual person as well as  population level.  The supportive SDOH risk factors that drove the index are categorized and stack ranked for an individual person using 19 of the 21 US Health and Human Services subcategories for SDOH. This AI algorithm includes model training from the CDC Social Vulnerabilities Index and Area Deprivation Index.

Retention Risk

An AI algorithm that can help health systems and health plans identify common data profile patterns of individuals that either churn to competitors or that are newly acquired patients and members. These profiles contain specific information to broaden your understanding of your patients and members for better and more relevant engagement strategies. In addition, an analysis is done to provide common data profile patterns and geographical lists for targeted outreach to new patients and members.

Redetermination Index

A proprietary algorithm the correlates income data for the individual or household to determine Medicaid, Exchange, or Private Insurance eligibility.  This product is specifically targeted to provide intelligence and insights in response to the end of the Public Health Emergency.  It can further be used to understand qualification attrition based on income since the information is updated monthly.
